Request for Proposal - Implementing a School-Based Mobile Vision Program for Chicago Public School Students
The Request for Proposals (RFP) for Implementing a mobile School-based vision program for Chicago Public School students will distribute funds under RFP DA-41-3045-12-2012-001. RFP DA-41-3045-12-2012-001 will fund two (2) categories: (1) Provision of Eye Exams and Prescription Eye Glasses, and Referrals for Follow-Up Care and (2) Quality Assurance and Evaluation Services. The RFP will be available online on December 20, 2012 at the following link: www.cityofchicago.org/health.
