Request for Proposal - Division of STI/HIV, HIV Housing Services
The Request for Proposals (RFP) for the Housing Opportunities for Persons with AIDS (HOPWA) program will distribute funds under RFP DA-41-2932-09-2012-001. RFP DA-41-2932-09-2012-001 will fund three (3) categories:
- Community Residence Operating Support,
- Housing Information Services and
- Tenant Based Rental Assistance (TBRA).
Funds are available to not-for-profit organizations located within the Chicago Eligible Metropolitan Statistical Area: Cook, DeKalb, DuPage, Grundy, Kane, Kendall, McHenry, and Will counties.
The RFP will be available online on October 31, 2012 at the following link: www.cityofchicago.org/health. Hard copies of the RFP can be obtained as of Wednesday October 31, 2012, from the Chicago Department of Public Health, DePaul Center, Room 200, 333 S. State St., Chicago, IL 60604. Proposal Due: November 23, 2012 by 4:00 PM
Contract Start Date: January 1, 2013
