Do I need an appointment?
We strongly suggest that you call first to make an appointment. Once an appointment has been scheduled with staff at (312.413.1852), you can be certain that the time reserved will be well spent. Scheduling an appointment will help to avoid a long wait, and ensure someone will be available to serve you. A phone call may also answer some questions, and will make service more efficient for you when you arrive.
What is the brochure policy?
1. Any agency or organization that has a contract with the Chicago Department of Public Health to provide STI/HIV/AIDS services is eligible to receive all available printed material with a limit of two hundred (200) copies of any specific title. Should more than 200 of any title be needed, we asked for a prior notice of four or more weeks, in writing.
2. Any other organization providing STI/HIV/AIDS services to Chicago residents may obtain printed material with limitations. The organization is limited to one hundred (100) copies of any specific title and to a limit of five (5) titles per request. In all cases the actual amount will be determined by availability of the material and the Center’s responsibility to maintain stock levels.
3. Any organization may request a sample packet that includes one of each title currently in stock. The materials obtained cannot be used in any fee-for-service activity or program.
4. Any resident of Chicago may request a copy of any brochure in stock.
5. Sample packets and single copy requests can be mailed to the requestor. All requests for bulk materials must be picked up at the program by the pre-approved CBOs. (Agencies and organizations requesting printed materials in bulk that have never dealt with the Chicago Department of Public Health for a special event, must submit their request in writing for approval 3-4 weeks prior to the event, which will also be subject to the availability of the materials and the Center’s stock levels).
