As of July 1st, 2012 the Chicago Department of Public Health (CDPH) has partnered with several different community based health clinics to provide services for primary care, children, woman, family planning and pregnant woman. CDPH will continue to provide important health services such as Immunizations, HIV testing mammography and more. You can click on the individual services at listed under each location to get specific hours and contact information.
