Emergency Preparedness Certificate

Emergency Preparedness Certificate for Fire Safety Director and Deputy Fire Safety Director Application Procedure

All Applicants Shall:

  1. Download Application Form from City of Chicago Web Site, or obtain the Application Form from the Bureau of Fire Prevention.
  2. Download the Emergency Preparedness Certification Study Guide or obtain a copy from the Bureau of Fire Prevention.
  3. Complete the Application including Section C (Notary Section).  Mail this to the Bureau of Fire Prevention along with the following:
    1. Two (1/½ inch by 2 inch) color photographs - passport type
    2. Check or money order in the amount of $35.00 made payable to the City of Chicago Department of Revenue.
  4. Take and pass a written examination.

 Applicants will be notified by mail as to the date, time and location of the examination.

Written Examination

All questions on the written examination will be taken from the Emergency Preparedness Certificate Study Guide.

 A current government photo ID must be produced at the examination site prior to the examination.

Renewal Procedure

 The Emergency Preparedness Certificate is valid for one year.  It is your responsibility to keep your certificate current. To apply for a renewal all applicants shall:

  •  Submit a new, completed, notarized renewal form
  • Make sure that the word Renewal is written on the top of the form.
  • Enclose two (2), current, passport-type photos.
  • Enclose check or money order made out to the City of Chicago Department of Revenue for the amount of $35.00.  The fee to activate an expired certificate is $50.00.

 The above list of documentation should be mailed to:

 The renewed certificate will be returned to the address listed on the application form.

 Please note that the replacement fee for a lost certificate is $15.00.
