2013 Military Make-Up Announcement for the 2006 Chicago Fire Department FF-EMT Examination

The City of Chicago, Department of Human Resources (DHR) will be administering a Military Make- Up for the 2006 Chicago Fire Department Firefighter-EMT examination. The Military Make-Up examination is offered to military personnel who were on active duty during the original application period (March 29-April 11, 2006) or examination period (May 25-May 26, 2006). All candidates must otherwise meet the minimum age requirement, possess a valid driver license and meet the education requirements as outlined in the original announcement.
Application Information
Military personnel must submit the following documentation in order to be considered for the examination:
1. A letter, addressed to the Commissioner of the Department of Human Resources, stating interest in taking the Military Make-Up examination. The letter must include current contact information (name, mailing address and phone number), and the signature of applicant.
2. The letter must include one of the following documents:
A. DD-214 (Member Copy 2, 4, or 6) showing dates of service and Box 24-character of service. Submit a readable photocopy only; no documents will be returned to you.
OR
B. Copy of Military ID and a letter from the Commanding Officer, on official letterhead, stating status of current armed forces enlistment, start date and character of service.
OR
C. Letter from the United States Veteran's Administration, on official letterhead, stating dates of service and character of service.
Written requests accompanied by the documentation must be received by the Department of Human Resources by the close of business day or postmarked by September 27, 2013. All documents must be sent to the following address:
