The Independent Living Program (ILP) is a comprehensive program that combines case management, assistive technology, the provision of in-home services (Personal Assistance/Homemaker ) and daily living skills training.
Individuals are referred for ILP services from various sources such as self referrals, social workers, discharge planners and others. An initial pre-screening is conducted to determine if the individual meets the basic criteria of the program. Eligibility is determined via an in-depth, in-home assessment conducted by MOPD's Independent Living Program staff.
This program explores all options necessary to help people with disabilities achieve their maximum level of independence through various approaches.
