Request for Proposal: Healthcare Provider Outreach and Education for Childhood Lead Poisoning and Prevention

June 1, 2018

Chicago Department of Public Health has released a Request for Proposal (RFP) for:  Healthcare Provider Outreach and Education for Childhood Lead Poisoning and Prevention. Aligned with Healthy Chicago 2.0, this program supports:  Reducing the percentage of children under 3 years with elevated blood lead levels is a core focus of the Healthy Chicago 2.0 plan.  This funding is specifically designated to reduce the percentage of lead-poisoned young children living in very low child opportunity communities by 10% annually. CDPH is providing funding to one (1) successful respondent through this RFP to partner with an organization to develop and conduct education and outreach activities to prevent lead poisoning targeting healthcare providers (e.g., medical office managers, nurses, medical assistants, physicians, nurse practitioners, physician assistants, etc.) in Chicago community areas where there is a higher risk of lead exposure.

All suppliers are required to register under the iSupplier portal at https://www.cityofchicago.org/city/en/depts/dps/isupplier/login-register.html prior to reviewing and submitting a proposal.  Upon registering, you may review and respond to the RFQ application here: https://eprocurement.cityofchicago.org/OA_HTML/OA.jsp?OAFunc=PON_ABSTRACT_PAGE&PON_NEGOTIATION_STATUS=ACTIVE
