Chicago Department of Public Health has released a Request for Proposals (RFP) for: School – Based Oral Health Program (Program). The Program will provide preventive oral health services to all students in the Chicago Public Schools from Pre-Kindergarten to 12th grade that provide a positive consent form for treatment. The children and adolescents in Chicago will benefit from improved access to oral health care with the goal of positively impacting health outcomes and school attendance.
Aligned with Health Chicago 2.0, this program supports: This RFP is related to Healthy Chicago goal of Strengthening Child and Adolescent Health, and to the overall goal of creating a city with strong communities and equitable access to resources, opportunities, and environments that maximize health and well-being. All suppliers are required to register under the iSupplier portal at www.cityofchicago.org/eProcurement prior to reviewing and submitting a proposal.
