A complete application must include pages 3 through 10 from the Special Event Permit Package and any necessary supporting documents. The completed application and fee must be submitted to the Department of Cultural Affairs and Special Events (DCASE) at 78 E. Washington St., Ste. 400 on Monday through Friday from 8:30am to 4:30pm. The application fee is acceptable in a check or money order made payable to the City of Chicago. DCASE has the right to refuse applications that are deemed incomplete. The time of receipt is determined by the clock located in Room 400, Chicago Cultural Center or the electronic date stamp on the received email or fax.
