Adult Day Services

Provides the senior the opportunity to interact with peers in a supervised community setting. Services that are provided by adult care service include: transportation, counseling, nursing services, health monitoring, assistance with activities of daily living, nutritious meals and snacks; and therapeutic activity programs.

Case Management

Case Management units are located throughout the city to help the elderly access in home services that will allow them to live independently in their homes and communities. Seniors may qualify for in home services after an in-home assessment of their needs is completed. Seniors must be over 60 years old and live within the city limits. All requests for case management services must be made through CDFSS's Information & Assessment Unit.

Guardianship

Guardianship services protects the legal rights of an elderly person who can no longer make decisions for him/herself in matters relating to financial planning, health, and housing. Guardianship procedures involve the appointment by a court of a legally responsible individual or agency on behalf of the individual. Client situations in which guardianship should be considered include mental deterioration, mental illness, or sever physical incapacity.

Heavy Duty Chore Program

Heavy Duty chore Service is a service which involves the arrangement of extensive cleaning for those clients whose living conditions pose a threat to their health and safety. Services include, but is not limited to:

  • Trash removal
  • Cleaning windows
  • Removing and cleaning heavy drapes
  • Cleaning cabinets and closets
  • Heavy duty washing of clothes
  • Washing walls
  • Moving and scrubbing behind heavy furniture
  • Thoroughly cleaning the oven and refrigerator
  • Scrubbing the bathroom

 

Home Delivered Meals

This program provides nutritious meals to older persons (60 years and older) who are frail and/or home bound because of illness or incapacitating disability or are otherwise isolated.

Homemaker Services

This program will provide a worker who may perform shopping and essential errands, prepare meals, assist with personal care (grooming, bathing, dressing), assist with self administered medication and perform routine housekeeping tasks (vacuuming, washing dishes, cleaning floors, laundry and changing bed linens) depending on the seniors needs.

Housing Relocation Counseling Service

This program provides housing relocation services to persons, who are homeless or at imminent risk of becoming homeless due to impending eviction or substandard housing conditions. The service does not include direct financial to individuals to obtain housing.

Legal Services

Legal Services provide assistance in resolving civil legal matters, and protecting and advancing the legal rights and interests of seniors. Lawyers and/or trained paralegal provide investigation, advice, counseling, litigation, representation, appropriate referral, advocacy and community education. The program does not handle criminal legal matters, or fee generating cases such as Personal Injury or Workman's Compensation.

Nursing Home Prescreening

This is a screening made by a case manager to assess the need for long term care placement. This must be done before a senior is admitted into a Skilled Nursing Facility (nursing home) or Intermediate Care Facility.

Respite Care Service for Caregivers

Respite care is a service for the senior's caregiver. This service will provide time away from a caregivers duties, allowing them to take a needed vacation, run errands or visit with friends and family. Respite services can range from a few hours a week to overnight care for a period of time.

Short Term Chore Program

This program is designed to assist individuals who are recuperating from an illness, a temporary medical disability or have been discharged from a medical setting. The services is for six (6) weeks and will provide temporary help until the senior is well enough to do the chores themselves. The activities include light housekeeping tasks, laundry, and shopping.
