Online Claim Submission

The deadline for submission of online and mailed claims was December 11, 2017. This deadline has passed and no more online and mailed claims will be accepted. For information about a claim previously submitted, you may contact (312) 270-2536. For security purposes, you will need either a Notice Number or Ticket Number to get information about a particular claim.
