Find a Neighborhood Health Clinic

The Chicago Department of Public Health offers many services. Neighborhood Health Clinics that provide services are located all over the city, and in your neighborhood.


Our Public Health clinics have Doctors, Certified Nurse Midwives, and Nurse practitioners’ that will provide services for Children, Adults, Pregnant Women, and Women seeking Birth Control or Pregnancy Tests.

Primary Care services treat anyone who is sick or ill, has a chronic disease and screening for TB, Lead Poisoning, High Blood Pressure, Cancer, Diabetes, Asthma, and, STI (STD). Primary care also includes immunizations and other services to keep people well.

Immigration exams and refugee services are available at selected clinics.

These services are available for residents of the City of Chicago based on a sliding fee scale ($0 - $128) based on your income. We also accept Medicare, Medicaid and private insurance. HMO patients are referred to their primary provider. We provide assistance in completing the All Kids application, Senior Care, and other government funded programs.
