Ethics Forms Filed by 2019 Election Candidates

 

The Governmental Ethics Ordinance requires City employees compensated at the rate of $76,450 for 2009 and certain City officials to file a 2009 Statement of Financial Interests in the Spring of 2009.  On or about March 1, 2009, the Board will send these forms to City employees and officials who, City records show, are required to file them with the Board.

NOTE: Some City employees and officials will not be required to file a Statement of Financial Interests at that time, but may be hired at an annual salary rate, or receive a raise, title change or appointment later in the year that makes them required to file.

Those required to file should be aware that:

  1. City law (Section 2-156-150(a) of the Municipal Code of Chicago requires that the 2009 Statement of Financial Interests be filed with the Board of Ethics before the close of business on Friday, May 1, 2009;
  2. those who do not file their Statement before the close of business on May 1, 2009, will have to pay a $20.00 filing fee;
  3. failure to file a Statement on or before May 31, 2009 constitutes a violation of City law that subjects the non-filer to disciplinary action, up to and including discharge from City service; and
  4. our office cannot accept incomplete forms.

Please note that anyone may print or download these blank forms, but the Board of Ethics will NOT ACCEPT for filing any form that is returned to the Board's offices electronically.  Only those forms delivered to the Board in person or by mail that contain hand-written signatures can be accepted by the Board of Ethics for filing compliance with the relevant provisions of the City's Governmental Ethics Ordinance.
