The City of Chicago's Senior Services Area Agency on Aging is in the process of developing our FY2025-27 Area Plan on Aging for submission to the Illinois Department on Aging (IDoA). The Chicago Senior Services  Area Agency on Aging annually submits the Area Plan on Aging to IDoA to request Federal Older Americans Act and State General Revenue funding for implementing programs and services for older adults and their caregivers. The Area Plan is as a planning document that includes a detailed summary of the needs of Chicago's older adult population as well as a delineation of the programs and services that the Chicago Senior Services Area Agency on Aging will offer to meet the needs of older adults within the proposed budget.  We are seeking input from older adults and their caregivers regarding service needs and gaps.  If you are 60 years of age and older or a caregiver for an older adult (60 years of age or older), please take click on the link below to complete this short survey.  Thank you for your time and valuable feedback.
